What is case management?

Case Management is an active process devoted to the co-ordination, rehabilitation care and support of people with complex, clinical needs and their families. It aims to facilitate their independence and improve their quality of life whilst acknowledging safety issues. (British Association of Brain Injury Case Managers 2013)

Our experienced Clinical Case managers have backgrounds which include occupational therapy, physiotherapy, nursing and social work. We provide a service to children, young people and adults in the North West of England.

Service includes:

  • Initial assessment.
  • Provision of individualised case management and care plans.
  • Provision of support plans and risk assessments.
  • SMART Goal Setting within Rehabilitation Programmes.
  • Identification of and liaison with other treating professional whether through NHS, Social Services Voluntary or Private sector.
  • Monitoring and evaluating the rehabilitation process.
  • Attending and chairing client and multi professional meetings, attending conferences.
  • Management of recruitment, induction and supervision of support workers.
  • Regular reviews with support staff.
  • Supporting the client and their family through the litigation process.
  • Family support and education.
  • Advocating on behalf of the client.
  • Brain injury education.
 

All clients’ needs are met in an efficient and friendly way. We aim to maintain the clients’ privacy, dignity, their individuality and are sensitive to their changing needs.
