Our case managers work directly with our clients
Heather qualified as an Occupational Therapist (OT) in 1989 and worked for the NHS for 18 years both as an OT and Brain Injury Co-ordinator. She has been case managing for many years opening her own company Total Case Management Limited (TCM) in 2008. Heather is a highly experienced Case Manager and is registered with the Health Professions Council, is a member of the British Association of Occupational Therapists (BAOT) and British Association of Brain Injury Case Managers (BABICM).
Jane is a registered general nurse, qualifying in 2003 and gaining a specialist degree in district nursing in 2006 and is a supplementary nurse prescriber. Jane brings to the role of case manager extensive experience of working in the NHS, community working and nursing home environment. She has managed a caseload of clients with complex health conditions including MS, MND and Huntingtons and has experience of undertaking CHC assessments & reviews and managing packages of care for clients within the community setting.
Jane also has extensive knowledge of working within a multi-disciplinary environment, she has excellent communication skills, problem solving and assessment skills which have been further enhanced by working as a functional nurse assessor and disability assessor for both PIP and ESA benefits claims. Working within a nursing home environment has also provided her with the opportunity to assist in recruitment of staff, management of rotas, liaising with agencies and performing staff supervisions. She has undertaken a variety of audits to ensure compliance to CQC standards within the home. Jane is registered with the Nursing & Midwifery Council and is a member of BABICM at registered practitioner level.
Sarah joined Total Case Management in April 2016 as a Brain Injury Case Manager. She qualified as an Occupational Therapist in 2007, working in the NHS for over 8 years. Sarah has a lot of experience of working with clients who have sustained a traumatic brain injury, managing their complex needs, including challenging behaviours, emotional changes and cognitive impairments.
Sarah is experienced in working closely with members of multi-disciplinary teams to support individual assessments and treatments. She has worked alongside social services, commissioning bodies and community rehabilitation teams to ensure the client has support, resources and funding required, in ensuring their complex needs are met. Sarah is a registered member of BABICM.
Louise is a registered nurse (adult) and health visitor with over 20 years experience working with children and adults. She has significant experience working with adults and children who have an acquired brain injury leading to complex care needs either through accident or birth trauma. Her specific clinical skills include tracheostomy, long term ventilation, gastrostomy, colostomy, epilepsy, limited mobility including quadriplegia and tetraplegia, and cerebral palsy. Louise is an autonomic dynamic practitioner who can work effectively in a team including working collaboratively with other health and social care professionals. Louise has excellent communication skills and can relate to clients, families and liaise with other health professionals to achieve the best possible outcomes for our client's. Louise is registered with BABICM.
A very experienced chartered physiotherapist, Greg joined Total Case Management in 2018. He has previously worked in the NHS for over 20 years, with vast experience managing community teams looking after people with complex illnesses, and more recently working as clinical lead physiotherapist at the NW ventilation unit.
With more than 25 years experience of working in adult services in community and hospital settings, looking after people with complex conditions and needs, Greg combines an holistic approach to healthcare with a drive to ensure the best possible outcomes for his clients. Greg is registered with BABICM as a Brain Injury Case Manager.
Taymon has worked with people with brain injuries since 2010, spurred on by the enjoyment of his work Taymon completed his degree in learning disability nursing and social work. Following completion of his degree Taymon has gone on to work in various settings as a nurse with people with acquired brain injuries as well as those with learning disabilities. Taymon is registered with the NMC as an RNLD as well as a registration with BABICM as a Brain Injury Case Manager.
Collette has worked in health and social care for the last 25 years and has managed complex services and teams, she has a registered managers award and will be applying to the Care Quality Commission to be the registered manager for TCM in the coming months. This role will entail being responsible for maintaining the quality of service provided by Total Case Management Ltd.
Collette’s previous experience of working as a Senior Team Leader makes her the perfect person to support the care staff who are managed by TCM ensuring their mandatory training, skills and knowledge are always up to date, Collette also carries out all supervisions and PDR’s for these teams.
Steve is an experienced Registered Accredited Detective, Joint Agency Achieving Best Evidence trained with vast experience of managing and investigating complex Children and Adult safeguarding cases, serious violent crime, sexual offences and offences committed by employees against their employers. Steve has 26 years’ experience as a Police officer.
Steve has managed Joint Agency investigations, teams of Detectives, Social Workers and Independent Domestic Violent Advocates enabling him to ensure vulnerable victims were provided with all the support, intervention and protection needed to make them feel safe, and to provide the victims with the best possible outcome and ensure appropriate action was taken with perpetrators.
Steve is a director of Total Case Management Ltd and he has taken on the role of Safeguarding and Personnel Investigator, investigating allegations made by and against staff. He has completed several investigations to date all of which have resulted in positive outcomes for the client and their team of carers allowing matters to be expeditiously resolved.
