4.1. With your permission information about you is shared with relevant team members and third parties such as, but not limited to, Solicitors, Physiotherapists, Occupational Therapists, SALT etc. the information is shared to enhance and promote your rehabilitation ensuring a holistic approach to your rehabilitation is achieved.

4.2. Information will only be shared with the parties in 4.1 once a consent form has been signed.

4.3. You can withdraw your permission at any time by sending an email to enquiries@totalcm.org.uk.

4.4. Other than as set out above, we will not disclose any of your personal information without your permission unless we are required by law to do so (for example, if required to do so by a Court Order or for the purposes of prevention of fraud or other crime.
