Clinical Case Manager - Frodsham

Head Office Location is in Frodsham, Cheshire 

Office and Hybrid working

Full time – 37.5 hours per week

Salary: Dependent upon experience

Are you an experienced Occupational Therapist/Nurse/SALT/Social Worker or Physiotherapist looking for a new challenge working with clients who have complex needs.  

We are looking for somebody who has experience in neurology including acquired brain injury, and/ or spinal injury to join our friendly team.

TCM Ltd provides a high-quality community-based rehabilitation service for individuals following an acquired brain injury or catastrophic injury. We are a friendly company who work across the North West of England and also into North Wales.  

Our ideal applicant would be centrally located to be able to travel to these areas.

The Role:

  • You will be responsible for assessing, planning, implementing and evaluating individualised case management / support plans alongside the client, their family and team.

Including:

  • Assessing for and referral to the multidisciplinary team.
  • Ensuring that thorough assessments are completed.
  • Ensuring full and comprehensive contemporaneous records are kept for every contact.
  • Completion of risk assessments.
  • Recruiting and developing packages of care individual to each client’s needs and ensuring that they run smoothly.
  • Producing costing reports for ongoing rehabilitation.
  • Develop good working and therapeutic relationships with the clients, their families, health professionals, litigation teams and charities.
  • Be able to produce written reports.
  • Liaison with a variety of professionals within health and legal settings.
  • Maintain full CPD portfolio.
  • Contribute to the development of TCM LTD.

About You:

  • Registration with relevant professional body with a minimum of two years post qualification preferably within the field of acquired brain injury/spinal injury or neuro disability.
  • Successful applicant will be required to register with BABICM on appointment.
  • You must have a Professional Qualification such as a degree or diploma in Occupational Therapy, Speech and Language Therapy, Nursing, Social Work or Physiotherapy.
  • You need to have proven working experience within Neurology/Brain Injury.
  • Community based work would be beneficial.
  • Have an interest in or knowledge of the medico legal sector.
  • Full access to a car is essential.
  • Training and formal support will be provided to the successful applicant.

Support Worker - Neston

Support workers required to work with a client in their own home - Neston Area

1 X 36 hours (3 x 12-hour shifts per week) – permanent

1 x 36 hours (3 x 12-hour shifts per week) – permanent

Shifts are worked during the daytime either 8am or 9am start and 8pm or 9pm finish.

We are recruiting on behalf of our client who is a 26-year-old young lady who has learning difficulties and Cerebral Palsy.

You would be working as part of an existing team and the role would include, but not limited to: assisting with all aspects of personal care which will include the use of a ceiling track hoist, full assistance at mealtimes, helping with socialisation, exercise and following and implementing prescribed therapy, improving independent living skills, finding new activities, planning structure and routine for each day which includes activities both in the home and in the community. There are additional duties such as cooking, shopping, and cleaning the house for the client.

Female Support Worker - South Warrington Area (P/T)

We are recruiting on behalf of our client and are looking for a female support worker to join an existing team, to work with a 16 year old young lady with cerebral palsy living in the south Warrington area. The client is non-verbal and completely dependent on others for her needs.

Your role will be to support the client with a range of learning activities (attend college), personal care, medication administration, regular physiotherapy and gastrostomy feeds. You need to be passionate about your work, and proactively provide a range of stimulating activities which will support the client’s cognitive and physical development.

The role includes assisting with personal care (which will include the use of a ceiling track hoist), helping with socialisation, exercise and completing prescribed therapy, improving independent living skills (including household chores – client’s washing and cleaning of client’s living area), finding new activities, planning structure and routine for each day, in a fun and innovative way.

Job Types: Permanent part time x 1, Casual part time x 1

Salary: £13 per hour during probation, £14 hour once passed

Expected hours: 20 – 25 hours per week
