What does case management mean to us?
Case Management is a proactive, person-centred service that coordinates rehabilitation, care, and support for individuals with complex clinical needs and their families. Its purpose is to promote independence, enhance quality of life, and ensure safety throughout the rehabilitation journey (British Association of Brain Injury Case Managers, 2013).
We provide services for children, young people, and adults across the North West of England.
